*PSI’s End User License Agreement does not allow for the contract in its entirety to be transmitted electronically. However, PSI does grant permission for pages 1 (Client Information) and 3 (Dog’s Profile) to be shared with your clients via e-mail if you prefer to receive this information prior to the initial meeting. For your convenience, copies of the pages that can be e-mailed have been placed in a separate folder on the CD labeled “Portions that can be e-mailed.” Under no circumstances should any portion of the Forms be posted online. Requires Microsoft Office Word. Note for MAC users: our forms are compatible with Microsoft Office Word for Mac. However, the CD Auto Run feature will not function when used on a Mac computer. This means that when you insert the CD into your CD Rom drive, the CD will not automatically launch to a menu screen. Contents of the disc can be accessed from your CD drive instead.
